Position Summary:
PLAN is seeking an enthusiastic compassionate Licensed Social Worker (LSW, LCSW, or LICSW) to join our team. We are looking for a candidate who has a passion for working with individuals with disabilities, providing support and advocacy, and collaborating with families and others who support beneficiaries.
The PLAN Service Coordinator will work with people of varying ages and wide-ranging disabilities to ensure that their trust funds are used to improve their quality of life, while protecting their eligibility for public benefits.
The ideal candidate must have proficient knowledge of public benefits (SSI, Medicaid, and public housing) and the ability to manage a substantial and growing caseload with varying degrees of responsibility. The Service Coordinator must have the ability to multitask, possess excellent assessment skills, and proficient interpersonal skills for developing rapport with beneficiaries and their families, case managers, attorneys, guardians, governmental and public and private agency personnel, and PLAN staff. The ability to work both independently and as a team member is essential.
Responsibilities include
- Initial meeting of new trust participants to explain the trust and use of trust funds, assess participants’ living arrangements, strengths, and weaknesses, and to develop a spending or life plan.
- Ongoing contact with trust participants
- Work with Trust Operations team to review and approve beneficiary requests, and weekly decisions on disbursements
- Resource development with the goal of giving participants ideas for using their funds
- Maintain current knowledge of public benefits, such as Medicaid, disability benefits and housing.
- Aid with program development and innovation in MA and RI
Qualifications
- Valid Social Work License in Massachusetts (LSW, LCSW, or LICSW) (dual license for MA/RI, a plus) (This position requires having a license in social work. Applicants without a license will not be considered.)
- Experience working with people over 65, people with physical disabilities, and people with mental health and intellectual challenges.
- Current Driver’s License
- Must have own car and be willing to travel.
- Familiarity with resources and supports for people with disabilities or in other human services
- Excellent interpersonal skills
- Ability to problem-solve and advocate
- Ability to communicate effectively and openly with a broad range of individuals and organizations
- Excellent organizational skills
- Ability to work independently and interdependently
- Ability to grow from supervision and direction
- Familiarity with computers and computer programs, such as Microsoft Office, Salesforce
- Integrity, positive attitude and self-direction
- A second language is welcome, but not needed
- Keen attention to detail
Salary negotiable based on experience and license level.
