Responsibilities:
- Assists residents and their families in connecting with community resources, healthcare providers, and other social services
- Conducts assessments and evaluations to identify psychological, financial, religious, and physical needs
- Develops and implements individualized care plans that meet the unique needs of each resident
- Collaborates with other disciplines to ensure comprehensive care and support
- Provides counseling and support services to residents and their families
- Maintains accurate and detailed records of resident interactions and services provided
- Education: Bachelor's of Science in Social Work or a related field
- Experience: Minimum 2 years of experience as a Social Worker
- License: LSW, LCSW or LICSW preferred but not required
- Skills: Strong communication, assessment, and social work methods and techniques; ability to work effectively with a variety of disciplines; knowledge of social systems and ability to implement pertinent state and federal regulations
