We are seeking a compassionate Licensed Practical Nurse (LPN) to join our clinical team.
In this role, you will provide direct nursing care, administer medications and treatments, monitor resident conditions, and support the delivery of high-quality, resident-centered care. You will collaborate with Registered Nurses, physicians, and other care team members, supervise and delegate tasks to Certified Nursing Assistants, and ensure accurate documentation within electronic medical records.
Key Responsibilities Include

Key Responsibilities Include
- Delivering individualized care per care plans and physician orders
- Monitoring and reporting resident condition changes
- Administering medications and therapeutic interventions
- Supporting care coordination and clinical operations
- Communicating effectively with residents, families, and team members
- Maintaining compliance with safety, infection control, and regulatory standards
- Graduate of an approved practical nursing program
- Active LPN license in good standing in Wyoming
- Current CPR or BLS certification
- Strong clinical observation, critical thinking, and communication skills
- Ability to work effectively in a team and manage multiple responsibilities
