Role: Licensed Practical Nurse (LPN) in Pediatric Home Health, providing focused, one-on-one nursing care in patients' homes.
Responsibilities: Support pediatric patients with skilled nursing needs, collaborate closely with RNs and clinical leadership, follow care plans, and ensure safety and compliance with CDC guidelines. Deliver care with confidence, build meaningful patient relationships, and utilize support resources when needed.
Qualifications & Skills: Valid Pennsylvania LPN license or multistate license, TB test or blood test, BLS CPR certification (in-person), valid driver's license, and experience or willingness to train in G-tube, trach, or vent care.
Key Duties: Provide safe, consistent care in a home setting, maintain communication with clinical team, adhere to safety protocols, and utilize provided PPE and support tools.
