The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs skilled nursing tasks in patients' homes, ensuring safety and adherence to care plans.
Key Responsibilities Include
- Delivering clinical services within scope of practice, aligned with state laws and care plans, in coordination with the healthcare team, following infection control protocols.
- Thoroughly documenting patient visits in electronic medical records and visit verification systems.
- Participating in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and other clinicians.
- Providing care in diverse home environments, respecting patient backgrounds and conditions.
- Current, unrestricted LPN license in the practice state.
- Valid driver's license, vehicle insurance, or access to transportation.
- Preferred: CPR certification, home care experience, multitasking ability, independence, strong communication and organizational skills, and flexibility.
