We are growing and looking for a compassionate, dependable, and skilled PRN Licensed Practical Nurse (LPN) to join our Home Health team. If you are passionate about caring for others and want to make a meaningful difference in the lives of seniors each day, we would love to meet you.
What You’ll Do
As a PRN Licensed Practical Nurse (LPN), you’ll play an important role in supporting the health, safety, and independence of our residents within their apartment homes throughout the community.
Your responsibilities may include:
- Medication management and administration
- Performing treatments and nursing procedures as ordered
- Monitoring resident conditions and completing assessments
- Responding appropriately to emergencies and resident needs
- Communicating changes in condition to the nursing team and physicians
- Assisting with activities of daily living (ADLs) when needed
- Supporting and guiding Home Health Aides and caregivers
- Maintaining accurate clinical documentation and records
- Promoting infection control and safety practices
- Creating a calm, respectful, and positive environment for residents

Physical & Work Environment
This role includes standing, walking, assisting residents with mobility and daily activities, and occasional lifting up to 50 pounds. You’ll work in a variety of home-like settings throughout our community while following safety, infection control, and resident care protocols.
