Job Description

The Community Health Worker (CHW) - Tribal Practices provides culturally relevant education on prevention and the promotion of holistic wellness for Indigenous peoples residing in Maricopa County. Additionally, is a liaison between, the NATIVE HEALTH Community Health and Wellness Department, Medical Department, and community members to facilitate access to services and improve the quality-of-service delivery. Including the coordination of services to improve diabetes-related health outcomes.

Knowledge, Skills, And Abilities

  • Facilitate group classes proficiently using a wide range of skills and techniques to help participants understand their common objectives and assist them to achieve them.
  • Understand how social determinants of health impact the community and participants.
  • Knowledge of Arizona Indigenous tribes’ practices of farming, gardening, and traditional foods.
  • Knowledge of client confidentially and client rights according to Health Insurance Portability and Accountability Act (HIPAA).
  • Ability to pass blood pressure and blood glucose testing competencies.
  • Strong problem-solving skills and assertive in challenging situations.
  • Highly resourceful team-player, with the ability to also be effective independently.
  • Creative thinker, who actively seeks opportunities and proposes solutions.
  • Demonstrates ability to write clear and concise reports of service activities and other programmatic reports.
  • Strong interpersonal skills including tact and flexibility to work effectively with supervisor, staff, and participants.
  • Advanced computer skills included MS office, specifically EXCEL, PowerPoint, Outlook, and Word.
  • Develop creative ideas, manage, and deliver diabetes-related education via digital content using a range of techniques, across channels including websites, social media, and Zoom platform.
  • Culturally competent; able to work with a diverse workforce and client population.

Qualifications

  • A Bachelor’s Degree in a health-related field is required.
  • Experience in community outreach or health education is a plus.
  • Knowledge of Electronic Health Records (EHR) preferred, NEXTGEN a plus.
  • Must be able to work irregular hours to include evenings and weekends as needed.
  • Must possess two years of specialized experience working within the American Indian/Alaska Native community.

Standard Employment Requirements

  • Must possess a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver’s license during employment. Must be insurable under NATIVE HEALTH’s liability auto policy.
  • Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
  • Must pass substance abuse testing upon employment and submit to random testing during employment.
  • Must provide an updated immunization (IZ) record at the time of employment.
  • Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
  • Must obtain Basic Life Support (BLS) certification within (90) calendar days of employment and maintain certification during employment.
