Job Summary
We are hiring a Community Health Worker, if you are passionate about providing support to MyCare clients by engaging them in healthier living patterns through proven 1:1 teaching and behavior modification strategies. The CHW builds individual and community capacity by increasing health knowledge and self‐sufficiency through development of a person-centered care plan and monitoring progress of interventions. The CHW serves as a liaison between health/social services and the client to facilitate access to services, improve cultural competence of service delivery, and to promote sustained health improvement and independence.

Job Functions
Essential Functions
- Act as a positive representative of Home Assist Health upholding the company’s Mission, Vision and Purpose.
- Performs as the onboarding specialist: engage and welcome referred clients to the MyCare program and team, complete the referral process, conduct and/or coordinate for the completion of the intake process and schedule first visits.
- Conducts in person home based assessments which may include:
- Environmental Safety and Fall Risk Assessment
- Social Determinants of Health Assessment
- Develop and implement person-centered home-based interventions (“MyCare” plans).
- Monitor and report on progress of MyCare plans and activities.
- Identify and recommend changes to MyCare plans and strategies for positive outcomes.
- Document all case related notes, updates and referrals in the Home Assist Health systems.
- Provides patient navigation support for utilization of health and social services, engaging patient in community resources and providing disease education materials.
- Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with recipient, their family, behavioral and general medical and dental health care providers, community resources and others to ensure that all services prescribed in the person-centered care plan are implemented.
- Provide health coaching, education and skill building for improved self-management of health and social needs.
- Triage phone calls, referrals and conduct home visits to support client’s transitioning from hospital to home, to address discharge plans, medication review, appointment adherence and social determinates of health.
- Enter patient services schedules and ensure services are billed according to current practice and procedure
- Advocate for and help clients get resources that would improve their well-being.
- Assist with other duties as assigned.
Qualifications
Education
- High School Diploma or equivalent
- Vocational training or healthcare specific college experience (i.e., MA, CNA, LPN, etc.) preferred
Specialized knowledge
- Care Plan Development and Strategy knowledge (Patient-Centered Care, Habilitation, ISP, etc.)
Skills & Abilities
- Requires the ability to read, write and speak effectively in English.
- Requires the ability to read, write and speak effectively in Spanish, native proficiency.
- Requires ability to professionally communicate with other Health Professionals at all levels.
- Must be proficient in computer skills including Microsoft Outlook, Word and Excel.
- Requires proficiency in time management, organizational skills, interviewing, and relationship-building
- Must possess exceptional customer service skills.
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.
Training/Certification/Licensure
Certification as a Community Health Worker through an approved CHW training program (can and will need to be obtained within a year from hire date)
- AHCCCS Direct Care Worker Training or current CNA
- DCW Developmental Disability training module
- Current CPR and First Aid Certifications required
Experience
- Required: Direct patient contact experience
- Preferred: Community health worker, Medical Assistant, Habilitation aide, CNA
Background Requirements
- · DPS Level 1 Fingerprint Clearance (or be able to obtain one within 90 days hire).
- · Criminal Background if applying for DPS Level 1 Fingerprint Clearance Card.
- · APS Registry Check at new hire and annually.
- Notarized Criminal History Affidavit at new hire and annually.
Other
- · Possess reliable transportation and current driver’s license.
- · Maintain current automobile registration and automobile insurance.
- · Negative tuberculosis (TB) test result on an annual basis.
Working Conditions
- Must be able to lift 50 lbs.
- May require extended periods of standing, sitting, or driving.
- Classified as Non-Exempt (FLSA).
