What are the responsibilities and job description for the Case Manager - Georgia Apex Program position at PINELAND BH/DD CSB?
Pineland BHDD is seeking a passionate and dedicated Case Manager to support students and families through the Georgia APEX Program. APEX is a statewide initiative funded by the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD) that increases access to behavioral health services in schools and promotes early identification, intervention, and care coordination for children and adolescents.
As an APEX Case Manager, you'll work directly with students, families, schools, and community providers to ensure youth have access to the resources, services, and supports they need to thrive academically, socially, and emotionally.
What You'll Do
Student & Family Support
- Assist students and families in identifying needs, goals, and barriers to success
- Connect students and families with community resources, healthcare services, and support programs
- Provide advocacy and guidance to help families navigate behavioral health and community systems
- Maintain regular face-to-face contact with students and families according to program requirements
Care Coordination & School Collaboration
- Collaborate closely with school personnel, behavioral health providers, families, and community partners
- Participate in multidisciplinary team meetings and treatment planning discussions
- Support coordination of care between schools and community-based providers
- Facilitate referrals and follow-up services to ensure continuity of care
- Help ensure students receive the right services at the right time to support academic and emotional success
Treatment Planning & Documentation
- Assist with development, implementation, and monitoring of person-centered treatment plans
- Complete required documentation, progress notes, and case records according to agency and DBHDD standards
- Maintain confidentiality and HIPAA compliance at all times
What We're Looking For
Required Education
- Bachelor's Degree in Social Services, Counseling, Social Work, Psychology, Child and Family Studies, Sociology or a related field
- Master's Degree is preferred, but not required
Preferred Experience
- Experience working with children, adolescents, and families
- Experience in school-based services, behavioral health, or community mental health preferred
Additional Requirements
- Valid Georgia Driver's License
- Ability to travel between schools, homes, and community locations
- Completion of required annual trainings
- Compliance with all DBHDD, HIPAA, and agency policies
