Your Role as Social Services Director

As the Social Services Director, you'll play an important role in supporting the emotional, social, and overall well-being of our residents.

Your responsibilities may include:

• Oversee and direct social services operations within the facility

• Conduct psychosocial assessments and maintain required documentation

• Participate in care planning and provide appropriate input for MDS assessments

• Advocate for residents' needs, preferences, rights, and well-being

• Collaborate with the interdisciplinary team during care conferences and care planning

• Assist residents and families with discharge planning and transitions of care

• Help residents and families access appropriate community resources and support services

• Build meaningful relationships with residents, families, staff, and community partners

• Support residents through admissions, transitions, transfers, and discharge

• Help address concerns and advocate for positive resident outcomes

• Support resident engagement and quality-of-life initiatives

• Maintain compliance with applicable state and federal regulations

• Provide guidance, leadership, and support within the social services department

• Perform other duties as assigned by management

Who We're Looking For

We're looking for someone who is compassionate, organized, and passionate about advocating for others.

Qualifications:

• Bachelor's degree in social work or a related field preferred

• Current Colorado social work licensure or eligibility preferred

• Previous experience in skilled nursing, rehabilitation, or long-term care strongly preferred

• Knowledge of MDS, care planning, discharge planning, and regulatory requirements preferred

• Strong communication and interpersonal skills

• Excellent organizational and time-management abilities

• Strong advocacy and problem-solving skills

• Ability to work effectively with residents, families, and an interdisciplinary healthcare team

• Compassionate, dependable, and committed to person-centered care
