Hospice Coordinator

Job Duties:

• Answer phones and coordinate details with pharmacy and DME partners
• Assist in management of IDT preparation
• Conduct Admission and Discharge audits
• Communicate with clinics, facilities, hospitals & other healthcare partners via phone
• Committed to providing excellent customer service.
• Ability to work in a fast paced environment & have the ability to be flexible.
• Assist Hospice Team members as needed with administrative tasks
  
  

Requirements:

• Computer proficiency
• Ability to work in a professional manner 
• Must work well both independently and in a group setting 
• Prior office experience 
• Professional appearance  
• Proficiency with Excel and Word 
• Problem solving skills and ability to multi-task 
• Excellent Organizational Skills
