Overview

Responsible for providing high quality administrative support to the bereavement staff. Responsibilities include the planning, implementation, maintenance, and provision of services necessary to patients, identified caregivers, hospice staff, and the community at large.

Responsibilities

  • Coordinates the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.
  • Plans for Bereavement Program development, expansion and refinement through annual evaluation of services.
  • Provides bereavement support to hospice personnel coping with work related grief through one-to-one counseling, special support programs and activities, and referral to community resources.
  • Provides bereavement information and referral services to callers from the community. Expands and maintains community resource list as needed.
  • Functions independently, but seeks and accepts guidance from other members of the interdisciplinary group, or from members of the community.
  • Participates in hospice activities such as in-service education, hospice personnel meetings, and relevant committees.
  • Advances professional knowledge and skills.
  • Actively participates in quality assessment performance improvement teams and activities.
  • Assists local leadership and Recruitment with the hiring of new Bereavement Coordinators. This may include reviewing resumes, conducting interviews and making recommendations for future hires.
  • Provides initial training for all new Bereavement Coordinators.
  • Provides periodic training for all Bereavement Coordinators.
  • Provides training on the use of Home Care Home Base as it relates to the Bereavement Program.
  • Conducts conference calls with all Bereavement Coordinators on a regular basis. Responsible for developing the agendas for each call and leading the calls.
  • Organizes and leads an annual meeting with all Bereavement Coordinators. The preference is a face to face meeting but will decide in conjunction with the COO if the meeting will be virtual.
  • In conjunction with the Compliance Officer, provides education on Conditions of Participation (COPs) and updates on GSH policy and procedure changes as they pertain to the bereavement program.
  • Should a Bereavement Coordinator position become open in a GSH location, the Lead Bereavement Coordinator will assist with ensuring a plan is in place for bereavement services and programs to continue as required. The Lead Bereavement Coordinator will decide in conjunction with COO if onsite supervision is needed during the vacancy.
  • Works with the Compliance Officer as needed on issues related to Bereavement Programs. In conjunction with the Compliance Officer, will conduct role playing experiences modeling the proper process for participating in State Survey interviews and answering Surveyor questions.
  • Dotted line responsibility to the COO regarding Bereavement Programs across GSH.
  • Accepts and performs other related duties and responsibilities as required.

Qualifications

  • Degree in Social Work, Qualifications of experienced clinical grief counseling – OR – degree (BD, MDiv) or equivalent theological degree from a college or university, and certified in grief counseling through an approved grief counseling training program.
  • Minimum 3 years’ experience in supporting families/caregivers and patients through death/dying issues and counseling individuals and/or groups regarding loss.
  • Hospice/experience preferred.
  • Strong leadership skills, as well as experience in successfully coaching and supervising staff to drive individual staff and overall program growth.
  • Excellent human relations skills as demonstrated by previous experience or achievements. 6. Excellent verbal/written communication skills.
  • Ability to accept different lifestyles, cultures, beliefs, and values.
  • Knowledge of and commitment to the hospice philosophy of care.
  • Can successfully pass background checks & drug screen
