PRN Physical Therapist (PT)

Location: Maple Heights, Ohio

Facility: Phoenix of Maple Heights

Position Summary:

Phoenix of Maple Heights is seeking a motivated and compassionate PRN Physical Therapist (PT) to join our rehabilitation team. This role provides flexible, as-needed coverage to support patient care and therapy services. The ideal candidate is committed to delivering high-quality, patient-centered care and working collaboratively within an interdisciplinary team.

Key Responsibilities:

• Evaluate residents/patients and develop individualized treatment plans based on clinical findings
• Provide skilled physical therapy services in accordance with physician orders and established care plans
• Document evaluations, progress notes, and discharge summaries using Net Health and PCC systems
• Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes
• Collaborate with nursing staff, physicians, and other therapy team members to ensure coordinated care
• Educate patients and families on therapy goals, home exercise programs, and safety measures
• Maintain compliance with all federal, state, and facility regulations and standards
  
  
  

Qualifications:

• Current license as a Physical Therapist in the state of Ohio
• Graduate of an accredited Physical Therapy program
• Strong clinical assessment and treatment skills
• Experience with Net Health and PCC preferred
• Excellent communication and interpersonal skills
• Ability to work independently and adapt to varying patient needs
• Previous experience in a skilled nursing or rehabilitation setting preferred
