Position Summary
The Behavioral Health Case Manager / Group Facilitator is responsible for providing direct client support through group facilitation, care coordination, and case management services in an outpatient behavioral health setting. This role works collaboratively with the clinical and operations teams to support client engagement, treatment adherence, and continuity of care while maintaining timely and accurate documentation.
Key Responsibilities
Group Facilitation
- Facilitate therapeutic groups in accordance with program schedules and treatment models
- Deliver structured, goal-oriented groups focused on mental health stabilization, coping skills, psychoeducation, and recovery
- Maintain a safe, engaging, and supportive group environment
- Manage group dynamics and address behavioral issues appropriately
- Complete group notes and attendance documentation accurately and on time
Case Management
- Serve as a primary point of contact for assigned clients
- Coordinate care with therapists, psychiatrists, and external providers
- Assist clients with accessing community resources (housing, employment, benefits, medical care, etc.)
- Support discharge planning and aftercare coordination
- Conduct client check-ins to monitor progress, attendance, and barriers to treatment
Client Support & Engagement
- Promote consistent attendance and participation in treatment services
- Communicate with clients and families (when appropriate) regarding treatment expectations and resources
- Identify and escalate client safety concerns to the clinical team promptly
- Support intake and orientation processes as needed
Documentation & Compliance
- Complete all required documentation in a timely and accurate manner in the electronic health record
- Adhere to all regulatory, accreditation, and agency policies and procedures
- Maintain client confidentiality in compliance with HIPAA and agency standards
- Participate in audits, chart reviews, and quality improvement initiatives as assigned
Team Collaboration
- Participate in treatment team meetings and supervision
- Communicate effectively with clinical, admissions, and operations staff
- Assist with coverage for groups or client support as needed
Qualifications
Required:
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related field
- or Minimum of 5 year experience in behavioral health, mental health, or substance use treatment
- Experience facilitating groups preferred
- Strong organizational, communication, and documentation skills
- Ability to work effectively in a fast-paced, team-based environment
Preferred:
- Master’s degree in a behavioral health-related field
- Licensure or licensure-eligible (LAC, LSW, LPC, LCSW, LMFT, etc.)
- Experience in outpatient or partial care programs
- Familiarity with electronic health records
