What You’ll Do
- Provide comprehensive primary care services, including assessment, diagnosis, and treatment
- Manage acute and chronic conditions across a diverse patient population
- Perform physical exams, preventative care visits, and health screenings
- Develop, implement, and adjust individualized care plans based on patient needs
- Educate patients and families on conditions, medications, and wellness strategies
- Collaborate closely with physicians and the care team to ensure continuity of care
- Accurately document patient care and maintain timely medical records
- Participate in quality improvement initiatives and help enhance care delivery
Qualifications and Experience
- Masters Degree in Nursing (Required)
- Nurse Practitioners Certification (NP-C) (Required)
- BCLS - Basic Life Support (Required)
- Department Specific Certifications (Preferred)
- Experience in primary care (Preferred)
- 3 years’ experience in supervision and independent decision making in major nursing areas (Preferred)
