Temp-to-Perm Opportunity | $46/hour
Are you a licensed Social Worker with care management experience looking for your next opportunity? We're partnering with a respected healthcare organization to hire a Care Manager – Social Work for a full-time temp-to-perm position.

Position Details- Pay: $46/hour
- Schedule: Monday-Friday, 8:00 AM-5:00 PM
- Assignment: Temp-to-Perm
- Full-Time: 40 hours per week
- Coordinate and manage care plans for participants with complex healthcare needs
- Participate in interdisciplinary team (IDT) meetings and collaborate with healthcare professionals
- Review clinical documentation for quality and compliance
- Coach and support field staff to ensure high standards of care
- Coordinate services including home care, rehabilitation, nursing home placement, transportation, and other community resources
- Monitor participant progress and modify care plans as needed
- Collaborate with physicians, specialists, discharge planners, and other providers to ensure continuity of care
- Maintain accurate documentation in accordance with regulatory and organizational standards
- Assist with staff training, competencies, and quality improvement initiatives
- Master's Degree in Social Work (MSW) from an accredited program
- Current, active New York State Social Work license
- Minimum of 2 years of care management or healthcare management experience
- Experience in home care, managed care, long-term care, hospital, or related healthcare setting preferred
- Experience working with the elderly or frail population preferred
- Strong communication, organizational, and documentation skills
- Supervisory or leadership experience is a plus
- Bilingual candidates are encouraged to apply
