Care Delivery Manager (RN) - Managed Long Term Care
💰 $67-$70/hr | 🕘 M-F, 8:30-5 (1 hr lunch)
📍 Non-Profit MLTC |  Contract 
📍 Bronx, NY

This isn’t a back-office role. This is where clinical leadership meets real-world impact.

We are partnering with a mission-driven Managed Long Term Care organization serving elderly, disabled, and chronically ill populations—and they need a Care Delivery Manager who knows how to run point on care, people, and process.

You will ensure care is coordinated, compliant, and actually working for the members it’s meant to support.

What you’ll own:

• Lead care delivery across the site—ensuring care plans are meaningful, executed, and driving outcomes
• Run the interdisciplinary team (IDT) process—meetings, service requests, and real-time coordination
• Oversee staff performance, coverage, and day-to-day clinical operations (this is hands-on leadership)
• Monitor documentation, audits, and compliance—keeping everything aligned with NYS and federal regs
• Partner across teams (clinical, transitions, facilities) to manage member movement and continuity of care
• Step in as escalation point—incidents, grievances, after-hours coverage, and problem-solving in real time

What you bring:

• Valid New York State RN license, required
• Managed Long Term Care (MLTC) experience - non-negotiable
• Minimum 2yrs Supervisory experience leading clinical teams in home care, LHCSA, CHHA, or similar settings
• Minimum 1yr experience working with a frail or elderly population
• Strong grasp of UAS, care planning, and regulatory expectations
• Ability to lead, pivot, and hold a high bar without losing the human side of care
