Essential Duties And Responsibilities

Essential Duties And Responsibilities
- Evaluates in-home care needs for concierge and premium clients
- Addresses emotional needs and concerns
- Coordinates medical services
- Serves as the primary point-of-contact for families and Clients for all clinical matters; conducts and directs meetings and calls with Clients and their families
- Coordinates with the Client Care team to prepare care givers and to ensure that the Plan of Care is being correctly executed
- Based off the needs of the Clients will, at times, need to perform skilled nursing visits during normal business hours and outside of normal business hours
- Provides Client education and teachings as needed
- Monitors and advocates for medical needs
- Enhances social and quality of life activities
- Establishes short-term and long-term plans
- Discusses difficult topics and complex issues
- Ad hoc projects as needed
- Bachelor's degree in health sciences, social work or nursing preferred
- 2+ years of experience in healthcare or human/social services industry
- Excellent communication abilities (written and oral)
