We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join a highly skilled Rehabilitation and Transitional Care team. This is an excellent opportunity for CNAs who are passionate about helping patients regain independence and recover following hospitalization, surgery, illness, or injury.
The ideal candidate will have experience providing direct patient care in rehabilitation, skilled nursing, long-term care, sub-acute care, transitional care, or hospital settings. You will work closely with nurses, therapists, and interdisciplinary healthcare teams to support patient recovery and ensure exceptional patient outcomes.
Position Details- Specialty: Rehabilitation / Transitional Care
- Shift: Nights
- Schedule: 5x8-Hour Shifts
- Contract Length: 13 Weeks

- Assist patients with activities of daily living (ADLs)
- Support patient mobility, ambulation, transfers, and repositioning
- Assist with feeding and hydration needs
- Provide bathing, grooming, oral care, skin care, and personal hygiene assistance
- Perform incontinent care and maintain patient comfort
- Obtain and document vital signs including temperature, pulse, respirations, blood pressure, pulse oximetry, and glucose testing
- Observe patients and promptly report changes in condition to nursing staff
- Maintain accurate documentation of patient care activities
- Follow infection prevention and isolation protocols
- Ensure a safe patient environment through proper transfer techniques and fall prevention practices
- Collaborate with nurses, therapists, and other healthcare professionals to support rehabilitation goals
- Active CNA Certification
- Current BLS Certification (American Heart Association preferred)
- Minimum 1 year of CNA experience
- Experience in one or more of the following settings:
- Rehabilitation
- Skilled Nursing Facility (SNF)
- Transitional Care Unit (TCU)
- Sub-Acute Care
- Long-Term Care (LTC)
- Acute Care Hospital
- Ability to work night shifts
- Strong communication and teamwork skills
- Previous travel healthcare experience
- Experience working with rehabilitation patients
- Familiarity with electronic medical records (EMR/EHR)
- Experience caring for adult and geriatric patient populations
