J OB SUMMARY:
Responsible for providing support for implementation and evaluation of systems used to identify high risk chronic disease patients. Assist the RN case manager to provide education and support, both within and outside of the Medical group practice, for the high risk chronic disease patient population.
MINIMUM QUALIFICATIONS:
High School Diploma required
License to practice as an LPN in the Commonwealth of Pennsylvania required
3 Years Experience In Nursing Required
KNOWLEDGE, SKILLS, & ABILITIES:
Strong Organization Skills Required
Strong communication skills both oral and written required
Ability to analyze data reports on patient outcomes required
Proficiency with Microsoft Office required
PREFERRED QUALIFICATIONS:
Experience With Solving Insurance Issues Preferred
