Job Description
- PURPOSE OF JOB : To provide professional nursing care in accordance with the Nurse Practice Act, to collaborate with the physician's medical plan, and to execute the objectives and the policies of the hospital.
- ESSENTIAL FUNCTIONS :
- Assesses, plans, implements, documents, coordinates, and manages patient care according to department and hospital standards and giving consideration to age-specific accommodations.
- Communicates and collaborates with patients, physicians, family members, and other healthcare disciplines in order to achieve excellence in patient care during hospitalization.
- Promotes patient safety through adherence to protocols, policies, procedures, evidence-based guidelines, and Joint Commission National Patient Safety Goals.
- Actively participates in and supports departmental and organizational quality initiatives and goals.
- Maintains clinical competency according to unit-specific standards.
- Assesses, plans, implements, documents, coordinates, and manages patient care according to department and hospital standards and giving consideration to age-specific accommodations.
- Utilizes the nursing process for patients assigned to his/her care.
- Maintains the computerized intraoperative record based on nursing assessment, the medical plan of care, and patient need.
- Provides patient care, which demonstrates a comprehensive knowledge of normal physiology and disease pathophysiology, appropriate nursing and medical interventions and a knowledge of the principles of growth and development over the lifespan.
- Implements patient plan of care based on evidence based practice.
- Insures appropriate completion of chart and other documentation including computer entry.
- Insures that no unapproved abbreviations are used when documenting on patient's medical record.
- Demonstrates appropriate skill in utilization of the computer for automated order entry, data retrieval, medication administration, nursing care planning and intervention and interdepartmental communication as determined by defined access levels.
- Demonstrates initiative and self-direction in completion of assigned responsibilities in accordance with department needs.
- Maintains competency in scrub and circulating duties and strives to improve upon areas of weakness as needed.
- Adheres to hospital and AORN standards in promoting patient safety and prevention of surgical site infections.
- Assists patients physically by providing physical support through pushing, pulling, lifting, reaching, turning, kneeling, standing, sitting and balancing, and psychologically by providing emotional intervention.
- Demonstrates ability to perform with clinical proficiency in crises and medical emergencies.
- Performs selected therapeutic interventions and diagnostic procedures according to department standards as ordered to include but not limited to:
- medication administration
- wound care
- point of care testing
- initiation and maintenance of oxygen therapy
- specimen collection utilizing mobile lab when appropriate
- phlebotomy (venipuncture, capillary), ABG's
- 12-lead ECG
- Insertion and maintenance of Nasogastric Tubes (NGT)
- Flushing and maintenance of central lines:
- *PICC lines
- *Single, double and triple central lines
- *Sub Q ports
- 2 patient identifiers will be utilized before performing these interventions and/or procedures
- Communicates and collaborates with patients, physicians, family members and other health care disciplines in order to achieve excellence in patient care during hospitalization.
- Utilizes effective interpersonal skills including interaction with staff through understanding, talking, hearing, seeing and listening.
- Involves patient in all aspects of the plan of care, serving as advocate when necessary.
- Consistently and continuously communicates, consults and shares pertinent information regarding changes in patient status and patient care.
- Uses interpersonal communication techniques that show a willingness to accept and give constructive feedback for the purpose of promoting professional excellence.
- Effectively communicates pertinent patient information to relief personnel and to post-operative care unit, and to external departments receiving patients post care.
- Demonstrates leadership skills in team building and interpersonal relationships to create a cohesive and productive environment.
- Recognizes the existence of cultural differences and utilizes appropriate resources available throughout the hospital to respond to the needs of the adult and geriatric patient.
- Follows staffing guidelines. In the interest of patient care, responds to changes or needs within the Vascular lab by cooperating with staff rotation and on-call assignments, including weekends and holidays.
- Promotes patient safety through adherence to protocols, policies, procedures, evidence-based guidelines and Joint Commission national Patient Safety Goals.
- Maintains safety precautions while providing patient care including infection control isolation, patient/bed positioning, restraint utilization, skin care, patient/specimen transport, physical environment, fall prevention, and work injury prevention.
- Identifies patient using name and birth date upon admission to the Cath/EP/Vascular Department
- Participates in "Time-out" prior to incision and documents appropriately.
- Maintains equipment in good working order, reports equipment failure, and initiates corrective action.
- Labels all medications as appropriate on and off the sterile field.
- Safely administers medications and treatments as ordered according to hospital policy and formulary using the appropriate principles of safe medication administration, 2 patient identifiers.
- Safely handles various types of drugs and body fluids using knowledge of appropriate standard precautions.
- Actively participates in and supports departmental and organizational quality initiatives and goals.
- Identifies professional goals, which reflect own strengths and weaknesses and seeks
- Attends and participates in meetings according to department standards.
- Supports and participates in department, hospital, and evidence based care quality initiatives, including core measures.
- Participates as a team member or in workgroups on department, hospital and/or ISLT evidence based care quality initiatives.
- Maintains clinical competency according to unit-specific standards.
- Demonstrates competency in maintaining specific technological skills required in the Vascular Lab and pre/post care recovery within department.
- Completes required annual hospital education as per hospital policy.
- Maintains technological competence through practice and ongoing participation in inservices and formalized professional conferences.
- Recognizes legal responsibilities and functions within limitations as specified by the Pennsylvania Nurse Practice Act.
- Acquires and maintains proficiency in IV administration.
- QUALIFICATIONS :
- Education: Graduate of an accredited School of Nursing. BSN required. Current licensure in the Commonwealth of Pennsylvania, Department of State Bureau of Occupational and Professional Affairs.
- Experience: Cardiovascular/Vascular or General Operating Room/PACU
- Other Skills: Certified in BLS. ACLS needed within 6 months of hire. PALS certification highly encourage within 1 year of hire. Interpersonal Skills.
