Certified Nursing Assistant (CNA) – Home Health – Per Visit / PRN
Penn Home Health Care is seeking compassionate and dependable Certified Nursing Assistants (CNAs) to provide quality care to patients in their homes throughout our service area.

Philadelphia, Delaware, Chester, Montgomery, Bucks, and Berks Counties, Pennsylvania
Position Type:
Per Visit / PRN
Responsibilities:
• Assist patients with activities of daily living (ADLs), including bathing, grooming, dressing, and toileting.
• Assist with ambulation, transfers, and mobility.
• Observe and report changes in patient condition to the supervising nurse.
• Reinforce patient safety and infection control practices.
• Provide compassionate and respectful care while maintaining patient dignity.
• Complete required documentation accurately and timely.
• Participate in care coordination and case conferences as requested.
Qualifications:
• Current Pennsylvania Certified Nursing Assistant (CNA) certification.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Ability to work independently in the home setting.
• Home health experience preferred but not required.
• Home Health Aide training/certification preferred.
