Responsibilities
- Provide one-on-one skilled nursing care in the home setting
- Perform comprehensive nursing assessments
- Administer medications and treatments as ordered
- Perform skilled nursing procedures in accordance with physician orders and the plan of care
- Monitor skin integrity and implement pressure injury prevention measures
- Assist with positioning, mobility, and participant safety
- Observe, assess, and promptly report changes in condition
- Coordinate care with physicians, therapists, family members, and the interdisciplinary team
- Complete accurate, timely clinical documentation
- Maintain compliance with agency policies, state regulations, and nursing standards
Qualifications
- Current Pennsylvania Registered Nurse (RN) License
- Current CPR Certification
- Reliable transportation
- Strong assessment and critical thinking skills
- Excellent communication and documentation skills
- Home care, rehabilitation, long-term care, or private duty nursing experience preferred, but not required
