Penn Home Health Care is seeking a motivated Occupational Therapist (OT) to provide patient-centered care in the home setting.
Service Areas:
Philadelphia, Delaware, Chester, Montgomery, Bucks, and Berks Counties, PA
Position Type:
Per Visit / PRN
Responsibilities:
• Complete evaluations and reassessments.
• Develop treatment plans focused on activities of daily living and functional independence.
• Provide therapeutic interventions and adaptive equipment recommendations.
• Educate patients and caregivers.
• Maintain accurate and timely documentation.
Qualifications:
• Current Pennsylvania Occupational Therapist license.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Home health experience preferred.
