Community Health Specialist Position Summary
The Community Health Worker (CHW) is primarily responsible for conducting patient screenings in the Emergency Department and may conduct assessments in other settings such as homes, community, or offices. Assessments include social needs, mental health, and substance use disorder surveys. Depending on the outcome of the assessment(s), the CHW provides culturally appropriate health education and information, provide outreach, help people get the care they need, give informal advice about health behaviors such as chronic disease self-management, advocate for individual and community health needs, and provide some direct services such as blood pressure screening. TidalHealth Community Health Workers follow best practices for assessing social risk factors for care. Following assessments, the CHW works with the patient to prioritize needs and helps address identified needs such as transportation, utility assistance, housing, financial assistance, health insurance coverage, and link to a primary care provider. The CHW conducts home visits and meets with patients in a variety of settings. The CHW is a key part of an interdisciplinary healthcare team that includes care coordinators, nurses, social workers, and providers. Activities will promote, maintain, and improve the health of patients and their families. The CHW serves as a liaison with community agencies and referral services, interfaces with external agencies, prepares and maintains timely and accurate documentation of all activities in accordance with established standards and procedures, as well as enter data in appropriate platforms such as Epic.
Community Health Specialist Position Requirements
Education
- High School Diploma or equivalent is required.
- Bachelor’s degree in relevant field is preferred.
License and/or certification
- Certified as a Nursing Assistant (CNA) is preferred
Experience
- Three (3) years clerical/administrative experience is preferred.
- Experience in a healthcare setting and medical terminology preferred.
- Experience working with Microsoft Office computer programs, practice management systems and spreadsheets preferred.
Community Health Specialist Schedule
- Varies schedule, primarily day shift.
- Evenings and weekends may be required as needed.
Community Health Specialist Benefits
At TidalHealth, team members working at least 36 hours per pay period based on 12-hour shift schedules or at least 37.5 hours for non 12-hour shift schedules and part-time team members working at least 30 hours or more on weekends only are eligible for benefits.
