Key Responsibilities
- Provide health education and wellness information to community members.
- Connect individuals with appropriate healthcare and community resources.
- Assist clients with navigating available health and social services.
- Conduct community outreach and support engagement activities.
- Help schedule appointments and coordinate service referrals when appropriate.
- Maintain accurate and confidential client records.
- Communicate with healthcare providers, community organizations, and service teams.
- Identify routine barriers to accessing services and help connect clients with resources.
- Support community health programs, workshops, and educational events
- Follow confidentiality, privacy, safety, and organizational procedures.
Requirements
- High school diploma or equivalent required
- Previous community outreach, healthcare, social services, customer service, or human services experience is preferred.
- Strong communication and interpersonal skills
- Excellent organizational and recordkeeping abilities
- Ability to work respectfully with individuals from diverse backgrounds
- Basic computer and documentation skills
- Professional, dependable, and community-focused
