Position Summary
Penda Aiken inc. is a staffing firm located in Brooklyn New York. Our client is a New York State Agency and is seeking a detail-oriented Nurse Practitioner (NP) or Physician Assistant (PA) professional to join their team. This position provides services at a 242-bed skilled nursing and rehab facility, including a 32-bed enhanced memory care unit for residents with cognitive impairment and Alzheimer's disease, located in Chenango County, Hwy 220, Oxford, New York 13830. The Department provides care to a population of Veterans, Veteran Spouses and Gold Star Parents.
The Department is seeking no more than two (2) Nurse Practitioners, or Physician Assistants. These positions are to provide services onsite at least five (5) days a week between the hours of 8:00 AM and 4:30 PM, Monday through Friday. They must also provide on-call medical coverage at all times (24 hours, 7 days a week) in the event there is no provider in the facility.
Primary Responsibilities
Deliver physician support staff services, including routine and as-needed resident evaluations.
Manage and execute comprehensive coordination of care for the facility's population of Veterans, Veteran Spouses, and Gold Star Parents.
Maintain prompt, accurate documentation in the facility's electronic medical record system.
Actively participate in interdisciplinary care planning processes.
Provide clinical coverage across the 242-bed facility, including specialized care within the 32-bed enhanced memory care unit.
Maintain on-call medical coverage readiness at all times (24 hours, 7 days a week) when no other provider is in the facility.
Help ensure adequate staffing levels are met to answer the facility's clinical demands, maintain continuity of care, and support quality outcomes.
Minimum Qualifications
Licensure: Active and unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of New York.
Schedule Availability: Commitment to working onsite five (5) days a week between 8:00 AM and 4:30 PM, Monday through Friday, plus full availability for the required 24/7 on-call coverage rotation.
Core Competencies: Proven experience navigating electronic medical record systems, handling care planning coordination, and treating geriatric populations or residents with cognitive impairments and Alzheimer's disease.
