As an Evening Shift Licensed Practical Nurse, you will play a vital role in providing exceptional care to our residents while promoting a positive and supportive environment. About the Role:

• Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs)

• Develop and implement individualized care plans in collaboration with the healthcare team

• Monitor and report changes in patient condition to the healthcare team

• Collaborate with other healthcare professionals to ensure seamless transitions and continuity of care

• Maintain accurate and up-to-date patient records, reports, and documentation

Responsibilities:

• Work closely with the evening shift nursing team to ensure a smooth transition of patient care

• Participate in unit-based quality improvement initiatives to enhance patient outcomes and satisfaction

• Maintain a safe and clean environment, adhering to Pearl Pavilion's infection control and safety policies

• Participate in in-service training and continuing education programs to stay current with industry best practices and regulations

• Demonstrate compassion, empathy, and respect for all patients, families, and colleagues
