Key Responsibilities
- Perform comprehensive physical therapy evaluations to assess residents’ functional abilities and rehabilitation needs
- Develop and implement individualized treatment plans focused on improving mobility, strength, balance, and overall physical performance
- Deliver high-quality therapy interventions that promote independence and enhance quality of life
- Document evaluations, treatments, and progress accurately in accordance with regulatory and facility standards
- Communicate effectively with residents, families, and interdisciplinary team members to ensure coordinated care
- Participate in care plan meetings and collaborate with the care team to support optimal resident outcomes
- Monitor resident progress and modify treatment plans based on clinical response and goals
- Maintain a safe, clean, and organized therapy environment
- Educate residents and caregivers on therapy goals, progress, and home exercise programs
- Current, active Physical Therapist (PT) license in the State of Wyoming, in good standing
- Previous experience in a skilled nursing, rehabilitation, or post-acute setting preferred
- Familiarity with electronic documentation systems (Net Health experience preferred)
- Strong clinical judgment, organizational skills, and attention to detail
- Excellent communication and interpersonal skills
