Polaris Rehab is seeking a dedicated and compassionate Certified Occupational Therapy Assistant (COTA) to join our rehabilitation team. In this role, you will work under the supervision of a licensed Occupational Therapist to provide therapeutic interventions that help residents improve their ability to perform daily activities and maintain independence.
This is a hands-on, rewarding position ideal for someone who enjoys building relationships with residents and supporting meaningful functional progress.
Key Responsibilities
- Implement individualized occupational therapy treatment plans developed by the Occupational Therapist
- Assist residents with improving activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility tasks
- Provide therapeutic exercises and interventions to support strength, coordination, and cognitive function
- Monitor resident progress and communicate updates to the supervising Occupational Therapist
- Document treatment sessions, progress, and resident responses accurately in the EMR system
- Educate residents and families on adaptive techniques and use of assistive equipment as directed
- Collaborate with the therapy and interdisciplinary care teams to support resident goals
- Maintain a safe and supportive treatment environment
- Assist in recommending and reinforcing use of adaptive equipment and modifications
- Follow all facility policies, safety guidelines, and regulatory requirements
- Graduate of an accredited Occupational Therapy Assistant program
- Current, active Certified Occupational Therapy Assistant (COTA) license in the State of Wyoming, in good standing
- Strong communication and interpersonal skills
- Ability to follow treatment plans and clinical direction accurately
- Compassionate, patient-focused approach to care
- Team-oriented mindset with willingness to collaborate across disciplines
- Experience in skilled nursing or rehabilitation preferred but not required
- Basic documentation and computer/EMR skills
