- Essential Responsibilities**:
- Provide services in the community where the consumer may need it,
shelter, store, etc.
- Provide outreach, support, referral to services and connection of
- Provide ongoing strengths assessment eliciting the consumer and or
wants, needs and preferences.
- With input from the client, their family and other members of the
services and activities. Monitor and document progress and concerns
adjusting the person-centered plan as needed.
- Advocate for client rights and community integration. Preserve
- Provide individual therapeutic interventions to assist in assessment
- Within scope of knowledge, provide resources and education to the
