JOB DESCRIPTION
Position Summary:
The Home Care registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community - based upon community standards of practice.
The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting.
The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery.
Work Schedule:
Every 6th-7th weekend, available up to 40 hours/month
Required Qualifications:
- Education, Experience or Equivalent Combination:
- Baccalaureate degree in Nursing required.
- Work experience as a registered nurse for at least two years
- Licensure/ Registration/ Certification:
- Current license to practice as a professional nurse in Minnesota.
- Knowledge, Skills, and Abilities:
- Must be able to function independently with minimal supervision.
- High level of sensitivity, assessment skills, technical-skills, and communications skills.
- Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
- Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.
Preferred Qualifications:
- Education, Experience or Equivalent Combination:
- Experience in home care desirable
- Experience in home care and or hospice desirable
- Must be able to function independently with minimal supervision.
- High level of sensitivity, assessment skills, technical-skills and communications skills.
- Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
