This is a full-time hybrid position for a Wound Care Nurse Practitioner, based in Jacksonville, FL. The Nurse Practitioner will assess, diagnose, and treat patients with complex wounds. Day-to-day responsibilities include performing wound evaluations, providing advanced wound care, educating patients and caregivers about wound management, and collaborating with interdisciplinary healthcare teams. The role requires proactive communication, thorough documentation, and a commitment to delivering compassionate care.

Qualifications
- Expertise in Wound Care, including advanced assessment and treatment of complex wounds
- Strong foundational skills in Nursing and Medicine
- Ability to educate patients and caregivers in wound management techniques
- Valid Nurse Practitioner license in the state of Florida
- Board certification in Wound, Ostomy, and Continence (WOCN) is preferred
- Strong interpersonal, organizational, and communication skills
- Proficiency in electronic medical records (EMR) systems
- Minimum of two years of clinical experience in wound or ostomy care is a plus
