Wound Care Nurse Practitioner (1099)
Skilled Nursing Facilities | Jacksonville, FL
Elements Wound Therapy is seeking an experienced, highly skilled Wound Care Nurse Practitioner to provide on-site care in Skilled Nursing Facilities (SNFs) throughout the Jacksonville area. This is a 1099 independent contractor role designed for a confident, procedure-driven NP who is comfortable managing complex wounds and practicing autonomously in a post-acute setting.
Our team specializes in advanced wound management, partnering closely with SNF leadership and interdisciplinary teams to improve outcomes, accelerate healing, and reduce avoidable hospitalizations.

Clinical Responsibilities
- Perform weekly (or more frequent as indicated) wound rounds in assigned SNFs
- Conduct comprehensive wound assessments, measurements, and staging
- Perform sharp/surgical and selective debridement
- Apply cellular tissue products (CTPs) and advanced wound care modalities when indicated
- Develop and adjust evidence-based wound treatment plans
- Manage complex wounds including pressure injuries, diabetic ulcers, venous/arterial ulcers, and surgical wounds
- Collaborate with facility staff, therapy teams, and interdisciplinary care providers
- Provide clear wound care orders and ensure implementation by facility staff
- Document all encounters accurately in EMR with appropriate CPT and medical necessity alignment
- Support facility goals related to quality metrics and rehospitalization reduction
Qualifications
- Active, unrestricted Florida ARNP license
- National certification (FNP-BC/FNP-C, AGPCNP-BC/AGNP-C, or AGACNP-BC)
- Minimum 1 year Nurse Practitioner experience in wound care REQUIRED
- Wound care certification strongly preferred (WCC, CWS, CWCN, etc.)
- Demonstrated experience performing debridement independently
- SNF or post-acute care experience strongly preferred
- Strong understanding of wound documentation, staging, and medical necessity
- Ability to work independently and manage a consistent rounding schedule
- Proficient with EMR and technology
