Physical Therapist (PT) – Home Health

We are currently seeking a Per Diem Physical Therapist to provide high-quality care across San Francisco County, California. This is a great opportunity for clinicians who value flexibility while still making a real difference in patients’ lives.

What You'll Do:

As a Physical Therapist, you will provide in-home evaluations and treatment, complete OASIS documentation, and collaborate with the care team to develop and manage patient care plans. You’ll also educate patients and families, support functional recovery and independence, and contribute to overall care coordination and agency needs.

Employment Type: Per Diem

Area Coverage: San Francisco County (travel required to patient homes)

Requirements:

• Licensed Physical Therapist in the State of California
• Preferably with minimum 6 months of home health PT experience
• Active BLS/ACLS certification
• Valid Driver’s License and auto insurance
• Up-to-date immunization records (COVID-19, TB test, Flu, Hep B, MMR, TDaP, Varicella)

Additional Requirements:

• Must be willing to travel to patient homes
• Must be able to lift 25 lbs
• Must have no health restrictions that would interfere with patient care
