Summary
We’re hiring a Per Diem Licensed Practical Nurse (LPN) to provide skilled home health care services throughout Casa Grande Valley area. You’ll work with an interdisciplinary team and deliver care that helps patients recover safely and independently at home.
Responsibilities:
- Deliver prescribed nursing care in the home under RN/physician supervision
- Monitor and report patient condition changes
- Ensure compliance with all applicable laws and standards

Requirements:
- Possess a Valid Licensed Practical Nurse license (LPN) in the State of Arizona
- Possess a Valid AZ Fingerprint Clearance Card
- Current and valid BLS (CPR) and ACLS certifications
- Negative TB test result and flu shot within the past year
- Up-to-date immunization/vaccination records including COVID-19, Hepatitis B, MMR, TDaP, and Varicella
- Valid Driver’s License with auto insurance
