Responsibilities:
- Conduct physical therapy assessments, skilled procedures, and ongoing evaluations in patients’ homes
- Complete OASIS documentation accurately and efficiently
- Collaborate with physicians, case managers, and caregivers to update care plans and ensure continuity of care
- Educate patients and families on therapy goals, home exercise programs, and available community resources
- Perform other related duties as assigned that may not be listed here but are essential to supporting patient care and agency operations
Requirements:
- Licensed Physical Therapist in the State of Arizona
- Active BLS/ACLS certification
- Valid Driver’s License and auto insurance
- Up-to-date immunization records (COVID-19, TB test, Flu, Hep B, MMR, TDaP, Varicella)
