The Social Worker will also coordinate respite stays and discharge planning, monitor care plans, and attend care conferences to ensure consistency in care across different settings. Addressing behavioral health needs, connecting participants with resources, and helping identify caregiver support needs are all core components of this position.
You’ll also assist participants with benefits access (e.g., housing, Medicaid, SNAP), and help participants with disenrollment and continuity of care. Keeping accurate and thorough records of all interactions in the electronic medical record ensures great participant care. The Social Worker role requires community-based work at partner facilities or in the homes of our participants and their families.
