POSITION SUMMARY
The Crisis/Supports Coordinator is part of the Education, Outreach and Welcoming (EOW) Team which is responsible for designing and carrying out the agency’s overall outreach efforts across the community including schools, community-based organizations, and all other places that youth congregate. In the role of Crisis Supports Coordinator you will work to support young people in navigating housing situations and accessing necessary supportive services, such as Case Management or Transitional Living services at Ozone House. The Crisis Supports Coordinator will then be responsible for conducting crisis and short-term interventions while working on connecting young people internally to Ozone House services or externally to community partners.
ESSENTIAL FUNCTIONS
Provide youth with crises & short-term interventions, referrals & linkages to Ozone House & other community services. Support youth, specifically on the waitlist, with navigating their current crisis by calling and checking in with them, meeting them in person, and meeting with them out in the community on a weekly basis; ensuring that their needs are getting met and they are supported
Provide supportive services such as obtaining vital documents, signing up for benefits, verification of homelessness, and connections to other housing and basic needs resources in the community
Facilitate referrals and linkages to Ozone House and other community services and resources. Monitor follow-through and/or success of these connections
Collaborate with other staff on the EOW team to support youth identified in programming at the drop-in center and at pop-up drop-ins, including connecting you with transportation for programming
Provide staffing coverage at the Drop-In Center once weekly
Actively participate in work groups, advisory committees, and other structures created for the purpose of aligning resources and collaboration
Attend Community Housing Prioritization (CHP) meetings to advocate for clients on Ozone House’s waitlist
Maintain program documentation including periodic progress reports, client files, and monthly data reconciliation in accordance with funder requirements and agency policy
EDUCATION REQUIREMENTS
Bachelor’s degree in social work or related field (required)
EXPERIENCE REQUIREMENTS
2+ years’ experience working with homeless, exploited or other high-risk youth.
Experience in crisis intervention and customer service.
Comfort with outreach work and meeting clients in their own homes or in the community
Demonstrated ability to effectively manage crisis situations
Proven ability to collaborate effectively with public systems and other providers
Experience engaging with diverse identities, including intersecting identities of race, ethnicity, socioeconomic background and sexual orientation, gender identity, and gender expression.
Demonstrated knowledge of and commitment to best practices including Positive Youth Development, Harm-Reduction, Housing First & Trauma Informed approaches/models
Effective written and verbal communication and interpersonal skills; ability to produce quality work within tight timeframes and simultaneously manage several projects; ability to participate in and facilitate group meetings.
Familiarity with word processing, spreadsheets, data entry and web navigation.
Demonstrated strong work ethic and ability to take initiative.
Demonstrates a high level of personal and professional integrity and trustworthiness.
Successful completion of pre-employment and annual background check
