Telephonic RN Case Manager- FUTURE ORDER with our top healthcare client
Remote | Michigan RN License Required
We’re hiring a Remote RN Case Manager to support a holistic, person-centered care management program for a diverse health plan population. This role supports one of the largest health insurers in Michigan and focuses on coordinating care for members with complex health and social needs.
Responsibilities:
- Lead a multidisciplinary team to deliver integrated care management
- Manage patients telephonically and via digital platforms
- Support members with chronic conditions, including the senior population.
Qualifications:
- Active Michigan RN license.
- 1 year of case management experience.
- 3 years in clinical, acute/post-acute care, and community setting required.
- 1 year in leadership
- Nursing diploma or associate degree required; BSN strongly preferred.
- Case Manager Certification or Chronic Care Professional credential preferred.
- Strong computer skills, 45 WPM, dual monitors and remote work experience.
