Description

SUMMARY

The Licensed Practical Nurse (LPN) at Opportunity Village is entrusted with delivering dedicated nursing services to enhance the health, safety, and overall quality of life for individuals served. This role involves active participation in assessment and evaluation processes, contributing expertise to the Person-Centered Planning (PCP) team, and implementing physician directives to ensure comprehensive care. The LPN will oversee and facilitate optimal nursing interventions, monitor medication management, and uphold high standards of care. Additionally, the LPN will be integral in formulating, executing, and sustaining individualized healthcare protocols in alignment with PCP objectives, state regulations, and Nurse Practice Act standards.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Provides services to all individuals assigned per the individual plans and seeks to assist others and team members when needed.
  2. Integrates in PRIDE room services as part of the team. 
  3. Alongside the team, LPN's are expected to support individuals in achieving their goals.
  4. Assists all individuals to perform and participate in scheduled activities and in Activities of Daily Living (ADL) as needed. 
  5. Completes required documentation that supports care and services provided, including but not limited to ID notes.
  6. Ensure equipment is clean and functioning. 
  7. Maintains a clean and safe work environment. 
  8. Anticipates safety needs for all individuals and team members. 
  9. Complies with all federal, state, and local regulations and requirements, as well as company policies and procedures.  
  10. Carry out habilitation assistance services as required by the participant's Person Centered Plan (PCP).
  11. Maintain appropriate documentation.
  12. Follow all medication administration procedures; documentation to be completed efficiently.
  13. Assist the Manager and Director with special projects.
  14. Supports and participates in department activities.
  15. Establishes cooperative relationships with team members to determine effective strategies to achieve individual and department goals.
  16. Assist the Supervisor Registered Nurse in developing appropriate Care Plans for each participant.
  17. Assist with intake and screening of prospective participants.
  18. Assist with initial nursing assessments with the RN.
  19. Assist with establishing charts for new participants.
  20. Administer medications and treatments per prescription orders, document them on medication sheets, and report adverse reactions.
  21. Receive medications from family as needed, ensuring all medications are logged and secured.
  22. Provide proper disposal of discontinued or expired medications and maintain a destruction log.
  23. Maintain narcotic sign-out sheet and count for controlled substances.
  24. Observe participants for condition change. Note in participant's charts and report book, communicate changes to family/caregivers or Physicians, if needed.
  25. Assess and triage emergency situations; provide appropriate treatment when possible, and contact emergency services (911) as needed.
  26. Assist with serving meals and feeding participants as needed. Monitor meals, ensuring that special diets are adhered to.
  27. Assist with loading and unloading buses as needed. 
  28. Teach proper techniques and sanitation as needed.
  29. Utilize appropriate methods for working with individuals with intellectual and or physical disabilities.
  30. Adhere to Opportunity Village's established policies and procedures.

  32. Some travel between work locations
  33. Report suspected Abuse, Neglect, Exploitation, Isolation, and Abandonment (ANEIA) immediately as mandated by state law.
  34. May be assigned other duties as needed.

 

SUPERVISORY RESPONSIBILITIES

This position does not directly supervise others.

Qualifications


SKILLS REQUIRED

  • Ability to communicate effectively with individuals, team members, and supervisors (verbal and written).

EDUCATION AND EXPERIENCE

  • Required: High School Diploma or equivalent.
  • Required: Current LPN license.
  • Required: Current CPI certification within 30 days of employment
  • Required: Current CPR and First Aid Certification within 30 days of employment
  • Required: Demonstrated proficiency in computer applications, including Microsoft Word, Outlook, and Excel.
  • Preferred: Minimum of two (2) years of experience working with people with disabilities.

  • Manual dexterity to operate office equipment and examine documents, records, and files.
  • Must be able to climb stairs as needed.
  • Flexibility and readiness to travel between work locations, be in transit, or in temporary workspaces, as needed.
