Responsibilities
- Assist individuals in daily living activities to promote independence and quality of life.
- Implement and monitor care plans tailored to each individual's needs.
- Administer medications as prescribed and ensure proper documentation.
- Observe and report any changes in clients' conditions or behaviors to the care team.
- Advocate for residents' rights and ensure a safe, respectful environment.
- Support clients in community integration activities and social engagement.
- Maintain compliance with HIPAA regulations and company policies.
Requirements
- Experience in healthcare or direct support roles preferred.
- Strong understanding of patient monitoring and medication administration practices.
- Ability to develop and implement individualized care plans effectively.
- Excellent communication skills for interacting with clients, families, and team members.
- Knowledge of resident rights and advocacy principles is a plus.
- Familiarity with home community care settings is beneficial.
