The Connected Care Community Health Worker provides information, resources, and care coordination to help older adult patients, and their families get the care they want and need. They work closely with patients’ primary care and behavioral health providers and communicate key information about what matters to patients in their life and care. They support patients to fill out the Advance Directive, reconcile their medications, remove fall hazards in a patient’s home, and connect them with community resources. This is a pilot program and the Connected Care Community Health Worker will work in partnership with primary care providers and the Connected Care team.

Job Specific Functions:

  • Provide Patient Support/Case Management & Patient Education:
    • Facilitate and support communication between the patient and the healthcare team, at most accessible locations: OCH health centers, home visits, and/or community settings.
    • Work with healthcare team to identify and overcome cultural and other barriers to behavior change.
    • Ensure the priorities and concerns of patients are identified and addressed. To include repeating in basic healthcare language diagnoses and instructions given by the PCP.
    • Perform assessment to determine best teaching methods and barriers to care that might affect patient’s ability to comply with treatment recommendations made by the PCP.
    • Acknowledge patient cultural traditions when providing education and care.
    • Provide basic education, self-management support, and goal setting for other chronic conditions.

Minimum Qualifications:

  • High School Diploma or GED.
  • Bilingual, English/Spanish.
  • 2 years’ experience in a health-related or social service-related field.
  • BLS/CPR Certification (must completed within 30 days of hire).
  • Valid driver's license and clean driving record.
  • Willingness to travel for events and between clinic locations, as needed
  • Experience giving presentations and provide instruction to patients and groups.
  • Able and willing to travel to: other OCH sites, community activities, trainings, home visits, and/or other locations as designated.
  • Familiar with medical terminology and the healthcare system including the Oregon Health Plan and Medicare.
  • Knowledge of community resources in OCH services areas, especially those available to older adults.
  • High-level computer skills with Microsoft Office applications, Word, Excel, Outlook, Epic, and other systems.
  • Knowledge of and compliance with HIPAA regulations.
  • Work independently, work with multiple projects and maintain a high degree of professionalism and confidentiality.

Preferred:

  • Advanced education such as graduate of an accredited Medical Assisting program.
  • 90-hour Community Health Worker certification or Certified Community Health Worker

Compensation:

  • Hourly/Non-Exempt: $20.00- $23.00, depending on experience.
  • Bilingual Differential: Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish.
  • Milage Reimbursement: In accordance with OCH policy.
