A bachelor's degree from an accredited four-year college with a major in social work, psychology, sociology or a closely related field.
OR
A bachelor's degree from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.

Regular and reliable attendance is a necessary component of job/position. Individuals required to use County vehicles and equipment must have a valid driver's license and be free of any major traffic violations for the last three (3) years.

Desirable Qualifications:

  • Two years of social service-related experience.

Expected starting salary is $34.62 to $46.23. Full salary range is $34.62 - $57.83 

Under supervision of a Program Manager or Supervisor, and within Federal, State and locally developed programs and policies, supports children diagnosed with a disability.  Identifies individual needs and works toward meeting these needs through county and community support services. The services help a person live as independently as possible in community settings and promote optimal health, independence, safety and community integration. Will be working within a multi-disciplinary team setting. Supports a diverse, respectful, and inclusive workplace. 


  • Conducts MnCHOICES assessments for children served through Waiver Programs and other related services.
  • Provides social services and case management services to families with children diagnosed with developmental, physical, or mental health disabilities. 
  • Coordinates services using a variety of funding programs/sources (County, State, and/or Federal) to meet the child's needs.
  • Educates individuals and their families of available community resources, information and referrals for services, rights and responsibilities under applicable programs.
  • Collaborates with other community agencies or organizations to gather information and coordinate services that meet the child's needs.
  • Develops individualized community support plans based on assessed needs and eligibility criteria.
  • Completes required documentation and contacts within established timelines to remain in compliance with State and Federal regulations.
  • Maintains accurate, confidential case records in accordance with State, Federal, and local guidelines.
  • Stays current on policy updates and program changes from the Department of Human Services (DHS) that affect children with disabilities and their families.
  • Utilizes state and county data systems (e.g., SSIS, MMIS, MnCHOICES) to document services, assessments, and case notes.
  • Performs other duties as assigned, ensuring flexibility in meeting departmental or child's needs.

  • Knowledge of a variety of programs and services for children with disabilities or related conditions, including knowledge of physical health, mental health, and co-occurring conditions.
  • Knowledge of the dynamics of individual and group behavior.
  • Knowledge of the principles, methods and techniques of social work.
  • Knowledge of social and economic problems.
  • Knowledge of community resources.
  • Knowledge of federal and state Social Service programs and laws.
  • Knowledge of data practices and confidentiality.
  • Skill in effective written and verbal communication.
  • Skill in the use of computers and technology.
  • Skills in individual and community advocacy.
  • Experience coordinating services for individuals and conducting assessments.
  • Ability to understand individual and family needs with empathy and objectivity.
  • Ability to exercise good judgment in recommending solutions for individual and family needs when utilizing conflict management and advocacy skills.
  • Ability to establish and maintain effective working relationships with individuals, co-workers, public officials, and citizens of the community.
  • Ability to organize efficiently and keep records systematically.
  • Ability to work with diverse populations.
  • Ability to serve as a liaison to community resources.
