Overview
The Mental Health Partnership with Schools Facilitator, under the guidance of the Program Supervisor, will be an integral part of a team that provides social and emotional support for the families of district students referred for intensive mental health services. This person will oversee the development of the family plan of care and facilitate the monthly Child and Family Team meetings.
This is a field based role with the main office being in Bellflower, CA and clients being in the local community. This role will work independently in the family’s home and in a variety of community and/or treatment settings.
Responsibilities
- Provide Wraparound services to children and families, supporting service coordination and implementation for each assigned client.
- Facilitate Child and Family Team Meetings
- Provide psychoeducation and support parents in dealing with the child's needs and problems.
- Provide linkage services
- Respond 24/7 to crises that may arise utilizing the Columbia Scale for assessing suicidality when necessary.
- Facilitate the implementation of the four-step Child and Family Team Process. Identify family strengths, underlying needs, and past traumatic experiences in planning. Utilize appropriate strategies and trauma informed interventions to assist families heal and achieve their goals.
- Develop and implement trauma informed client treatment plans specific to the client’s diagnosis in coordination with the Clinician, supervisors and team.
- In coordination with other Wraparound personnel and the Child and Family Teams (CFT), initiate and complete all documentation.
- Cultivate contacts and relationships with Family Team members and outside agencies and individuals that have both direct and indirect bearing on the case. These will include: County Social Workers, County Eligibility Technicians, School Personnel, Placement Workers or Referring Agencies, Regional Center Personnel, and Mental Health Professionals
- The MHPS Facilitator is expected to maintain a flexible approach and schedule to meet the family’s needs and model and promote strength-based practices consistent with the Wraparound philosophy
Qualifications
- A Bachelor’s degree in Social Sciences, Social Work, Psychology or related field.
- Minimum one-year experience working with high-risk youth.
- Negative Mantoux PPD Test/chest x-ray not more than one year prior to commencing work and every three (3) years thereafter confirming employee is free of tuberculosis.
- Must have CPR and First Aid certification upon hire and maintained throughout employment.
- Record clearance free of criminal convictions.
- Valid driver’s license, reliable vehicle, clean driving record, and current auto insurance
- An ability to work as a treatment team, be flexible, dependable and have good communications skills, both verbal and written.
- Exercise good judgment, maturity, and problem-solving ability in situations where limited guidelines exist.
- Must have general computer skills and working knowledge of Microsoft Office Suite and trainable in other computer systems.
- Must have a helping philosophy that aligns with Olive Crest's philosophy, values, mission and treatment philosophy to support abused children.
- May spend significant time driving as part of regular duties.
- Ability to climb stairs, lift 30 pounds, drive long distances in traffic, bend and squat
- Must be able to walk on uneven surfaces, walk up and down stairs and hills.
PREFERRED QUALIFICATIONS:
- Familiarity with, and willingness to utilize, community resources.
- Comfortable building relationships with culturally diverse families.
- At least two (2) years full-time equivalent experience (paid or unpaid) in delivering services in the mental health field.
