OVERVIEW
Provide therapeutic services to children and adolescents enrolled in the Wraparound program. Develop and implement client treatment plans/case management.
JOB DUTIES AND RESPONSIBILITIES
- Develop a therapeutic relationship with each client while adhering to agency’s treatment model. Assess individual needs and assist in meeting needs. Develop treatment plans and coordinate mental health services.
- Complete MediCal documentation as required by the Dept of MH and within the Department of Mental Health timelines.
- Participate in Wraparound Child Family Team Meetings a minimum of 1x per month.
- Serve as liaison between program and community treatment resources.
- Maintain detailed records of significant contacts, incidents, relationships, treatment team meetings, group and individual sessions, and case management activity. (I.e., Mental Health Documentation).
- Complete reports and submit to Treatment Coordinator/Director by due date.
- Document discharge planning.
- Attend individual as well as group clinical supervision weekly
- Attend IEP's, trainings, seminars and conferences related to role, as necessary.
- Document input and follow through on treatment plans. Review with staff for implementation and filing.
- Provide clinical consultation and training to staff as necessary
- Provide relevant clinical information to CSWs and probation officers in dealing with child behaviors. (Weekly treatment team meetings).
- Available to handle crisis situations that may arise during working and non-working (on call) hours.
REQUIRED QUALIFICATIONS
- Must have a master’s degree in social work, Clinical Counseling or Marriage and Family Therapy AND license OR associate number/BBS registration.
- 1-2 years relevant experience with an at-risk population
- Must speak fluent Spanish
- This role is field-based and will require driving to clients; must have reliable transportation, valid California Driver’s License and auto insurance.
