Job Description
Briar is seeking a detail-oriented MDS Nurse (RN or LPN) to join our long-term care team. This role is responsible for completing and coordinating the Minimum Data Set (MDS) process, supporting accurate care planning, regulatory compliance, and reimbursement. The MDS Nurse partners closely with nursing leadership and the interdisciplinary team to ensure high-quality resident care and compliance with CMS, Medicare, and Medicaid requirements.
This position is ideal for a nurse with experience in long-term care, skilled nursing, or MDS coordination who is organized, clinically strong, and passionate about resident outcomes.
Key Responsibilities
- Complete and coordinate MDS assessments in compliance with CMS, Medicare, Medicaid, and state regulations
- Ensure timely and accurate MDS submissions to support Medicare/Medicaid reimbursement and PDPM
- Develop, review, and update individualized care plans
- Collaborate with the interdisciplinary team (nursing, therapy, social services, providers)
- Monitor residents for changes in condition and ensure appropriate documentation
- Participate in care plan meetings and communicate assessment findings
- Support survey readiness and maintain regulatory compliance
- Educate and support clinical staff on MDS documentation standards
- Maintain accurate documentation in the electronic health record (EHR)
Qualifications
- Active RN or LPN license
- Experience in long-term care or skilled nursing preferred
- Prior MDS experience strongly preferred
- Knowledge of CMS regulations, Medicare, Medicaid, and PDPM
- Strong organizational, analytical, and communication skills
- Ability to manage deadlines and multiple priorities
- Experience with EHR and MDS software
