Key Responsibilities
- Complete accurate and timely MDS assessments and related documentation.
- Develop and update individualized care plans based on resident assessments.
- Coordinate interdisciplinary team (IDT) involvement in the care planning and MDS process.
- Monitor regulatory compliance related to RAI processes and resident documentation.
- Ensure accuracy of Medicare, Medicaid, and insurance documentation to support appropriate reimbursement.
- Review resident charts for changes in condition, medication updates, and treatment needs.
- Participate in quality improvement initiatives and audits.
- Communicate effectively with nursing staff, therapy, social services, dietary, and administrative teams.
- Provide education and support for staff regarding assessment and documentation standards.
- Maintain strong working knowledge of RAI, PDPM, and state specific requirements.
Required:
- Active Ohio RN or LPN license in good standing.
- Experience in long term care or skilled nursing facility (SNF).
- Working knowledge of MDS 3.0, care planning, and regulatory guidelines.
- Strong attention to detail and documentation accuracy.
- Excellent communication, organizational, and teamwork skills.
- Proficiency with electronic medical records (EMR) systems.
- Prior MDS/Resident Assessment Coordinator experience.
- Familiarity with PDPM and reimbursement practices.
- RAC CT certification (or willingness to obtain).
