We are looking for an experienced, talented, and compassionate Performance Improvement Specialist for a full time position.
The Performance Improvement Specialist will be responsible for the quality and compliance review of coding, OASIS, and Plan of care review for Home Health
What Will You Do
- Review SOC, ROC, Recert, Transfer, and Discharge OASIS assessments for accuracy and CMS compliance.
- Review medical records to identify disease processes that need a diagnosis code assigned and apply appropriate diagnosis codes following guidelines and conventions in MatrixCare
- Review OASIS and ensure consistency between OASIS scoring and clinical narrative.
- Identify missing elements, scoring issues, or inconsistencies that require clinician revision.
- Ensure documentation supports PDGM clinical grouping and Medicare standards.
- Review plan of care and if needed make recommendations related to the patient’s plan of care based on clinician documentation
- Collaborate with the clinical managers regarding trends and outcomes for the clinical teams
- Ensures Agency compliance with federal and state laws and regulations including the Medicare Conditions of Participation and Benefits Policy Manual
- Assists in the preparation of accreditation/certification surveys
- Assists, conducts and/or coordinates performance improvement studies as requested.
- Ensures agency compliance with all federal and state laws as well as adhering to the Norwell Visiting Nurse Association, Inc. Corporate Compliance policy in the prevention of fraud and abuse on behalf of the agency.
- Comply with HIPAA regarding privacy and security of medical record
- Performs other responsibilities as assigned.
What we’re looking for in you:
- Strong clinical skills
- Strong communication skills, both verbal and written
- Proficient in Microsoft Office (Excel, Word, PowerPoint)
- Able to work under pressure
- Able to problem-solve and troubleshoot quickly and effectively
- Strong organizational, decision-making, and time management skills
- Able to be flexible and dependable.
- Graduate of an accredited School of Nursing or Therapy Program
- OASIS certification
- ICD-10 coding certification
- Current MA. Professional License (in good standing).
- (5) years of home health coding experience
- (5) years of OASIS experience
