Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs, medication administration, and overall condition, reporting any changes promptly
- Prepare nutritious meals and assist with meal planning to support dietary needs and preferences
- Maintain a clean and safe environment through light housekeeping tasks such as cleaning, laundry, and organizing
- Support clients with mobility using equipment like Hoyer lifts or transfer belts, ensuring safety during movement
- Provide companionship and emotional support to reduce feelings of loneliness or isolation
- Follow care plans meticulously, documenting patient progress and communicating with healthcare professionals as needed
Qualifications
- Previous experience in home care, assisted living, nursing home settings, or group homes is highly preferred
- Knowledge of HIPAA regulations to ensure patient privacy and confidentiality
- Certification in CPR and First Aid is required; additional training in dementia, Alzheimer’s, or hospice care is a plus
- Ability to work with individuals with developmental disabilities, autism, or memory care needs
- Strong observational skills for patient monitoring and recognizing signs of distress or health issues
- Physical ability for heavy lifting and assisting with transfers using equipment such as Hoyer lifts
- Excellent communication skills to collaborate effectively with clients, families, and healthcare teams
